Meals on Wheels of Central Maryland Partners with NextFifty Initiative in Funding Through a Grant to Support the Together in Care Program
BALTIMORE - July 9, 2018 - PRLog -- Meals on Wheels of Central Maryland is proud to announce that it has been chosen as one of the grant recipients from NextFifty Initiative, a Denver-based nonprofit Foundation dedicated to funding innovative, mission-driven initiatives that improve the lives of older adults and their caregivers. The funded project is Together in Care, which is designed to ease the return back home after a stay in the hospital.

Stephanie Archer-Smith, Executive Director of Meals on Wheels of Central MD, said of the grant, "We are so pleased that NextFifty Inititative, through its support of our Together in Care Program, helps to bring awareness and vital funds to our effort to improve health outcomes among the vulnerable senior population we serve. Using state of the art technology, health and safety screenings are being done at the time of meal deliveries. Our goal is to improve seniors' health and to catch problems early. This can avoid costly nursing home and hospital stays."

"We recognize how important it is for us to stay in touch with individuals after they have been discharged from the hospital and to do what we can to see that they are receiving nutritious meals, that their homes are free of safety hazards, and that we hear about any changes in their health that call for attention.  Working closely with Meals on Wheels of Central Maryland in our Together in Care Program, we are able to have this valuable type of connection," said Dr. Dan Hale, Special Advisor to the President of Johns Hopkins Bayview Medical Center and Director of the Healthy Community Partnership.

In 2017, Meals on Wheels of Central prepared over 1 million meals providing nutrition to elderly and homebound individuals in Baltimore City, and Anne Arundel, Carroll, Harford, and Howard counties, as well as limited areas of Montgomery, Prince George's and Frederick counties. In Baltimore City, 294,233 meals were delivered to 1,086 clients.
