Third Quarter Medicaid Market Share Trends by Mark Farrah Associates

Total Medicaid enrollment included nearly 85.8 million Americans, representing a year-over-year (YOY) decrease of over 2.2 million enrollees, as of September 30, 2023 (3Q23). This decrease in enrollment is primarily due to the conclusion of the public health emergency (PHE) on March 31, 2023, when many state health departments initiated “unwinding” plans of renewals and redeterminations for coverage. Mark Farrah Associates (MFA) analyzed third quarter Medicaid enrollment trends from its County Health Coverage™ database tool.

Highlights include:

  • California was one of only thirteen states with a YOY increase in Medicaid enrollment, even with a negative quarterly change for 3Q23.

  • Oregon, Nevada, and Hawaii also experienced YOY increases, with Oregon experiencing a YOY increase of about 9%. Nevada and Hawaii both experienced notable quarterly increases as well as YOY.

  • Texas lost over 487K Medicaid members YOY, including a decrease of nearly 476K in 3Q23; over 86K of the quarterly decline was from Harris County.

  • Florida Medicaid program declined by 552,000 YOY members, with nearly 48,000 enrollees losing coverage in Miami-Dade County in 3Q23, alone.

  • Alaska experienced the largest decline, based on percentage of enrollment from 2Q23 to 3Q23, with 18.9% of its Medicaid members losing coverage, representing just under 50,000 covered lives.
